Hello, my name is David Andrew Green Jr., I was in a car accident roughly 2 years ago, which was caused by a PTSD episode from my time in the Army. After the accident I was rushed to the hospital for emergency surgery, because was unable to feel or move my legs this was due to the fact that my back had been broken and a spinal cord injury. They performed a fusion of 4 vertebras and put in a 8 inch rod in my spine. My spinal cord injury is located from T4 through T8 of my spine is considered incomplete. The car accident left me as a paraplegic and confined to a wheelchair with the need a full-time caregiver. I spent several months in the hospital and rehab facility to help me learn to coupe and deal with the changes, I would experience since my life would be much different now that I'm in a wheelchair. I'm very thankful for my significant other, Amanda, who has taken on the responsibility of being my full-time caregiver. She lost her job shortly after my accident, due to me needing a caregiver and assistance on a daily basis. Since the accident we have struggled to pay our bills and keep our life going. We have limited our expenses and downsized in everyway possible. We have used up all of our savings and cashed in our retirement accounts to pay our bills. We are asking for assistance to help with paying for rent, medical needs, and daily life necessities.

Here is a little bit of my history. We have 3 children between Amanda and I, which are all girls! I love them very dearly and would do anything for them. They have been very understanding and supportive to the changes of our life. I was born in Mississippi; lived in Alabama; and then moved to Prescott, Arizona after my parents divorce. I attended high school there and ended up getting my GED, so that I could join the Army. Prior to the accident, I was attending the University Of Phoenix as a full-time student for a Bachelor's Degree in the Science of Business and had just completed the last of my classes the month before my accident. I have always worked or held some kind of employment and not being able to work as been difficult on me. I have held positions with Charles Schwab, Chase Bank and served in the Army and Arizona National Guard. I joined the Army in 2007, because of the 9/11 attacks on our country and the support of the American way of life. My MOS in the Army was a 19D, which is a Cavalry Scout and I went to basic training in Fort Knox Kentucky. After basic training, I was stationed in Fort Carson, Colorado with 1-10 CAV Squadron of the 2nd brigade of the 4th Infantry Division, as a Buffalo Soldier from 2007 to 2011. In the army I held several different positions from HUMMWV driver, Bradley driver, gunner on both, and team leader and also worked the squadron's command sergeant major. We were deployed to Iraq in 2008 to 2009 and served in two different campaigns of the war in Iraq. My unit was stationed any many different locations in Iraq and we were moved around Iraq every couple of months and had daily missions outside the wire. During my deployment I developed PTSD and had other injuries. After returning home, I struggled to get back into the mindset of a normal lifestyle. After finishing my first contract with the Army in 2011, I decided to join the Arizona National Guard, because of the birth of my first daughter in June of 2010. My first wife and I were divorced shortly after that. In the Arizona National Guard, my MOS changed to 92Y, which is a supply specialist and served with Charlie Company 1-158th Infantry Division as a Bushmaster Soldier from 2011 to 2013. During this time I also worked full-time with Chase Bank as a personal banker, then business banker, and then a manger, up until 2014, when I took a position with Charles Schwab as a retirement specialist. While working I also went to college as a part-time student. I left Charles Schwab in 2016 to focus on my degree and finish school. I finished my classes with University of Phoenix in December of 2018 and had my graduation ceremony in March of 2019. Amanda and I meet in 2016 and were friends for a couple of years before we started dating. Her 2 daughters from her prior marriage became daughters of my own and seems like they have been mine since day one. I have known them from the young ages of 3 and 4. All of our birthdays are in September except for my one daughter. Amanda and our girls are the source of my motivation and what keeps me going to fight for a better quality of life for myself. They are all very supportive and understanding of how much our life has changed, since the accident. I'm very thankful for Amanda stepping up to all the responsibility of taking care of me and her willingness to do so. I don't feel that many women in her situation would stick around or be willing to do some of things that are required for my daily care.

We have been working hard to give daughters a normal life and keep it as stable as possible, since my accident. We have been trying to get to get to a point that will allow Amanda to find a work from home job or a position that allows her to be able to work a few hours here and there to help provide extra income for our family. This has been difficult and hard to do, because of all the assistance that I need on a daily basis and with all the doctors appointments, physical therapy, and schedule of my daily needs. We are asking for any assistance that you can provide to help us from losing our apartment, provide for medical needs, daily necessities, and to be able to get to a point were we are in a better position.
